The Service Bureau
employment forms are in Adobe Acrobat Reader format
(pdf files). You will need Adobe Acrobat Reader 4.0
or higher to read/print the files. You may download

free of charge from the Adobe web site.
APPLICATION
PACKAGE-ALL STAFF
For flying staff,
the following eight items must be included in your application
package. For non-flying staff, complete items 1, 4,
5, and 8.
- Resume
- Copy of FAA Airman Certificate(s).
- Copy of First Class Medical Certificate.
- Three letters of recommendation. The letters must
be from any individuals who can attest to the pilot's
flying skills; for non-flying applicants their business
skills, by having observed them over a sustained period
of time.
- Fully completed Employment Application-All Staff.
